This notice describes the feedback form on ClinicManagers, an independent project exploring practical resources for outpatient practice management.

What you choose to share

The form asks for your experience. Your role and email address are optional. Please leave out patient, employee, and other confidential information. No attachments are requested.

Where your response goes

The form sends your response to this website’s server, which uses Resend to email it to contact@clinicmanagers.com. Resend processes the response and any optional email address you include; see Resend’s privacy policy. You stay on this site when submitting. If the form is unavailable, you can use the email link instead.

How it is used

Your response helps inform which resources to build. If you include an email address, it may be used to follow up about your experience. Sharing feedback does not sign you up for launch updates or other marketing emails.

Storage and questions

This website does not save form responses in a database or in your browser’s local storage. The server temporarily keeps submission identifiers and fingerprints to limit repeat submissions; it does not retain the response text in that record. Responses are processed by the email provider and stored in the project’s inbox.

The form’s application code does not log message contents or email addresses. This site does not add analytics or session-recording scripts. This notice does not describe the hosting or email provider’s own logs or retention practices.
